If you wish to apply for River Valley Metro ADA paratransit service, you have three different options. The quickest way is to complete the application online. The application begins below. If you prefer a paper application, you can select the link to the right and print out a paper application and mail it to the address on the form. If you do not have the capability to print out the application, please call (815) 935-1403 and request an application be mailed to you.

Here is the Application Process

  1. Complete Application (either online or paper copy and submit)
  2. When the application is received, you will be contacted to set up an in-person interview. River Valley Metro will provide free transportation to and from the interview.
  3. Once the interview is completed and Metro will work on completing the process with the Professional Verification, but you are eligible to start riding River Valley Metro immediately.
  4. You will be eligible to ride the system for 21 days or until the application process is complete. If the application process is not complete on day 22, you are able to continue riding until a determination has been made.
  5. If your eligibility has been denied and you wish to appeal this decision, you will be able to ride until the appeal process is complete.

If you use the online application and have the capabilities, please print out the Medical Release form on this page and bring with you to your appointment. If you cannot print out, bring your physicians information with you to the interview.
